Charge Nurse LVN/RN- Monday through Friday 2pm-10pm or 10pm to 6am. Primary purpose is to provide direct nursing care to the residents and to supervise day to day nursing activities performed by nursing assistants and medication aides. Full time with benefits and sign on bonus. Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program. Certified Nurse’s Aide- 8-hour shifts. Full time with benefits and sign on bonus. Provide residents with care and assist in their daily living needs and recreation.
Hospice RN: Full-Time, Administers Medications and treatments. Assists in formulationtreatment programs and documents accordingly. Develops, implements and revises Nursing Care Plans.
Hospice Social Worker: PRN position. Provides emotional support and counseling for patients and families from intake through bereavement. Performs initial assessment and develops recommendations for the plan of care.
