Employment Opportunities

Housekeeping– Full time and PRN available. The housekeeping position is responsible for a variety of housekeeping duties. These duties are necessary for providing the resident population and the facility with a clean and sanitary environment. Shift is 7:30am to 4pm.

Certified Nurses Aides- Now hiring CNAs with a sign-on bonus. Full time and PRN shifts available. Open shifts include 6am – 2pm, 2pm – 10pm, and 10pm – 6am.

Charge Nurse – Now hiring a Charge Nurse (LVN/RN) with sign-on bonus. Full time and PRN shifts available. Open shifts include 6am – 2pm and 2pm – 10pm.

Dietary Aide– PRN with opportunity to become full time after first 90 days. The primary purpose of this position is to provide assistance in all food functions as directed/instructed and in accordance with established food policies and procedures.

Activities assistant– Part-Time. Leading, directing and instructing residents in activities, and the ability to work with nursing staff, social service department and follow directions of the Activity Director.

Care Plan Coordinator/Associate MDS Nurse– Lutheran Sunset Ministries is currently seeking an RN/LVN for the Care Plan Coordinator/Associate MDS Nurse position. Office Hours Monday – Friday. Great benefits to include: Medical, dental & life insurance, PTO, retirement plan, college tuition assistance, and free gym membership. Responsibilities: -Care Plans- Create and update care plans for all residents. Work closely with all staff to design complete personalized care plans for each resident. Schedule conduct and lead care plan meetings. -Satisfaction Surveys- Supply receptionist with list of resident/RP/POA surveys and gather data for QA meetings. Address deficiencies with department heads to improve facility quality/resident satisfaction. -MHMR Meetings- Coordinate, conduct, and carry out MHMR visits or virtual visits or calls. -Weights- Responsible for gathering resident weight logs from appointed staff member(s) who weigh residents according to orders. Work in conjunction with MDS Coordinator to communicate information. Work with Registered Dietitian and facility dietary manager to implement dietary recommendations into resident care plans. -Transportation- Manage transportation staff member to ensure resident appointments are made and carried out. Assist/guide with planning transportation. -Supplies- Review and code medical supply invoices and send to finance department on a timely basis. Communicate discrepancies to finance department or supplier. -MDS- Responsible for completing all private pay MDS for facility. Work closely with MDS Coordinator and correspond necessary information for compliance.

Infection Control/Treatment Nurse– Lutheran Sunset Ministries is currently seeking a RN/LVN for the Infection Control/Treatment Nurse position. Office Hours Monday – Friday. Great benefits to include: Medical, dental & life insurance, PTO, retirement plan, college tuition assistance, and free gym membership. Responsibilities: Infection Preventionist/Infection Control- Must possess or be willing to complete online Infection Preventionist certification provided by and approved by the CDC within 6 months. Responsible for surveillance of antibiotic use in the facility. Monitors and logs infections monthly, tracking and trending. Assists with insuring policies and procedures are followed related to infection control. Provides in services and training when necessary. Treatments- Provides wound care treatments for advanced wounds, hard to heal wounds and pressure sores. Provides/assists with training to Charge Nurses when necessary. Consults with wound care specialist physician when necessary. Orders special wound supplies/dressings. Skin Reports- Completes weekly skin report with advanced/difficult wounds and communicates to MDS coordinator. Immunizations- Provides immunizations & TB tests to residents when applicable. Enters new admission vaccination records into resident EMR. Responsible for administering TB tests to staff when applicable. Keeps log of employee TB tests and provides orders for chest x-ray when necessary. Orders/logs yearly flu vaccines for Sunset Home. Coordinates yearly flu shots for staff and residents. Manages Preventative Health Records (TB Tests/Flu Vaccines) Staff and Residents – Receives resident information from Admissions Coordinator and follows through insuring data is reflected accurately. Receives staff information from Human Resource Director and administers new employee TB tests. Directs and participates in administration of flu vaccines for staff and residents. Nurse Schedule Manager – Works closely with nurse scheduler to ensure bonus forms and payroll time is entered into schedule. The nurse scheduler reports directly to this position.

RN Alternate Administrator-Hospice– Lutheran Sunset Ministries is currently seeking a RN for Alternate Administrator for our Hospice/Home Health. Full time with benefits. Duties :  Coordinates the clinical services provided by the individuals providing care to hospice patients. Assists in the development of policies and procedures for clinical services. Assures that a comprehensive assessment of the patient is performed, that a Plan of Care is developed, and assures physician orders are obtained prior to implementation. Gives direct patient care as needed according to the Plan of Care and determines nursing measures based on individual patient need. Documents a detailed report of care given which reflects the Plan of care implementation. Administers treatments prescribed under the plan of care within Hospice Sunset guidelines. Provides training and supervision to Home Health Aides, LVN’s, and other IDT members as needed and appropriate. Prepares and receives a patient report which will assure continuity of care. On call Experience. Work in oncology, geriatrics or Hospice are required. At least one year experience as an RN. Home Health experience desired. 1 year management experience Education: Currently licensed as a Registered Nurse in Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact.
